Athletic forms for 2024-25 school year are now available online through skyward:

• Athletes enrolling in grades 7^th -12^th MUST complete (1) online UIL Forms, SISD athletic forms via skyward registration and (2) a Sports Physical Exam.

• A Sports Physical Exam & Medical History Form MUST be completed prior to participating in ANY practice, game, in-season or off-season athletic program. Sports Physicals for the 2024-25 school year, MUST NOT be taken prior to April 1^st, 2024

Note: Sports Physicals dated, prior to April 1^st, 2024, will not be accepted for the 2024-25, school year. We recommend all student athletes to renew their 2024-25 sports physicals between April 1, 2024 and the end of the current school year.  Sports Physical Exams are to be completed by a physician of choice EVERY school Year. 

• Sharyland I.S.D. will ONLY accept Sport Physicals completed in the United States using ONLY the approved UIL Forms.Non UIL athletic physical forms will not be accepted!

COMPLETED physical and medical history forms are to be RETURNED only to schools athletic training office, or your campuses athletic secretary’s office.   Do not turn in completed physical forms to the front office, nurse’s office or coaching staff.  Please return to the school that you will be attending during the 2024-25 school year.

 We recommend you keep a copy of the completed athletic physical for your personal records.
